Establishing Bulk-Billed School-Based Health Services in Regional Victoria

Operational blueprint for establishing bulk-billed primary healthcare clinics in regional Victorian secondary schools inspired by the SchoolCare model.

Governance Framework & The School-Practice Memorandum of Understanding (MOU)

Operating a medical clinic on educational premises requires clear legal and operational separation between healthcare delivery and school administration. A formal Memorandum of Understanding (MOU) establishes:

  • Clinical Independence: The medical clinic operates under the clinical governance of an accredited General Practice (RACGP Standards 5th Edition), completely independent of school management.
  • Medical Records Segregation: Clinical records are stored within an independent, encrypted cloud/offline practice management system. School principals, teachers, wellbeing staff, and Department of Education personnel have zero access to medical files.
  • Clear Division of Responsibility: The school provides dedicated, compliant consulting rooms, while the medical practice provides qualified clinicians (Specialist GPs and practice nurses), medical indemnity, diagnostic equipment, and clinical consumables.

Physical Room Specifications & Infection Control Compliance

Consulting rooms within school grounds must comply with AHPRA, RACGP, and AS/NZS 4187 infection control standards:

  • Acoustic Privacy: Solid-core doors and acoustic insulation to ensure consultation conversations cannot be overheard from corridors or waiting areas.
  • Clinical Hand Hygiene: Dedicated clinical handwashing sink with elbow- or sensor-operated taps, liquid soap dispensers, and paper towel holders.
  • Examination & Procedural Area: Clean examination couch with disposable paper sheeting, adequate examination lighting, and emergency equipment (oxygen, bag-valve-mask, adrenaline, and AED).
  • Clinical Waste & Sharps Disposal: Dedicated biohazard waste bins and puncture-resistant sharps disposal units compliant with Environmental Protection Authority (EPA) guidelines.

Clinical Scope of Practice in School Settings

School-based clinics provide comprehensive primary care across the full spectrum of adolescent medicine:

  • Mental Health & Emotional Wellbeing: Anxiety, depression, self-harm, eating disorders, family breakdown, and preparation of GP Mental Health Treatment Plans (MHTPs).
  • Sexual & Reproductive Health: Confidential contraception counselling, STI screening, emergency contraception, and pregnancy options counselling.
  • Chronic Disease Management: Paediatric asthma reviews, eczema management, allergy plans, and epilepsy oversight.
  • Neurodevelopmental Support: ADHD medication monitoring, coordination with paediatricians, and collaboration with school learning-support teams.
  • Preventive Care & Catch-Up Vaccines: Routine adolescent vaccinations and health assessments.

Financial Sustainability: The Medicare Bulk-Billing Architecture

To ensure zero out-of-pocket costs for students and families, services are 100% bulk billed under Medicare. Leveraging the tripled bulk billing incentive for children under 16 and Commonwealth concession card holders, alongside standard consultation items (Level B item 23, Level C item 36, Mental Health items 2715/2717), creates a financially viable, sustainable operating model that supports ongoing rural clinical outreach.
